Under direction of the Program Manager of Crisis Residential and within federal, state and local guidelines performs entry-level to routine habilitative and rehabilitative patient care services work. Work involves providing health and personal care services to patients; observing, reporting, and documenting patient conditions and behaviors; and intervening in medical and/or psychiatric emergencies.

Responsible for monitoring clients, providing household upkeep, and program oversight functions.

Education, Training and Experience (including licensure and certification)

A high school diploma or equivalent required. Client care experience for 1 year preferred. Experience working with mental health population strongly preferred.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

- All job specific training must be completed within 30 days of hire.

Knowledge, Skills, and Abilities

  • Knowledge of therapeutic methods and techniques in the rehabilitative therapy area of specialization, and of mental illness and mental disabilities.
  • Ability to provide rehabilitative therapy and to communicate effectively.
  • Understanding and practice of Trauma Informed Care principles.

Essential Duties

  1. Completes all scheduled cleaning duties for the nightshift including but not limited to disinfecting all surfaces, sweeping, mopping, cleaning out the refrigerator, taking out trash, and doing the dishes.
  2. Completes hourly and 30 minute checks on all clients as scheduled and document client activities on provided check forms; intervenes when clients are awake and provides supportive listening and encouragement to individuals; wakes clients up on time each morning.
  3. Preps all meals scheduled for the night; preps meats, make lunch, chop veggies, cook breakfast, and defrost meats as scheduled for the next shift; maintains current food handler's certification.
  4. Files progress notes in order; enters AQoL scores into spreadsheet; audits charts each night and flags any needed signatures, credentials, or missing paperwork; breaks down discharge charts and sends to medical records when corrections are complete; ensures paperwork for new client binders is always stocked.
  5. Performs inventory on food prior to grocery shopping; makes shopping list; performs inventory on all supplies including office, group, and cleaning supplies; completes supply request forms and submits to manager as needed.
  6. Supervises self-administration of medication.
  7. Provides courteous, professional services at all times; attends all scheduled clinical team meetings; meets with supervisor for monthly individual supervisions.
