Child and Adolescent Wraparound Facilitator

Under the direction of the Child and Adolescent Program Manager and within federal, state and local guidelines performs Wraparound/Intensive Case Management (ICM).  Work involves developing and maintaining long-term contact with youth and families, service providers for medical, social, educational, and related service needs; and support members who will be on the youth and family's team.

The Wraparound Facilitator should be an empathetic, creative, and resourceful individual. They should be flexible and committed to supporting children with behavioral challenges or various mental health diagnoses. All children and families deserve access to high quality services, and we strive to provide this support to those we partner with. As a Wraparound Facilitator, you will be working collaboratively with families, their support systems, and other professionals or community partners to develop individualized, strength-based plans of care. The Wraparound planning process addresses the reasons why individuals behave certain ways both within and outside of the family's home and formal human service systems.

Education, Training, and Experience

  • Graduation from an accredited four-year college or university with major course work in a human service field as required to qualify as a Qualified Mental Health Professional (QMHP).
  •  Minimum of (1) year related experience is preferred.
  •  Experience may include participation as a member of an Interdisciplinary Treatment Team in developing and implementing individualized program plans for MH consumers.
  •  Preferred experience in working directly with children and/or adolescents and their families

Registration, Certification, Licensure, and Other Qualifications

  • All NWIC Wraparound facilitation training must be completed within one year of hire.
  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver's license and automobile insurance.

o  Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.

  •   Successful completion of all position-specific training within thirty (30) days of employment is required.

Knowledge, Skills, and Abilities

  • Knowledge of community resources, of case management delivery systems, and of program policies and procedures.
  • Ability to assess youth and family's needs, coordinate individuals' services, communicate effectively, and to train others.
  • Has the ability to work independently and make decisions but seeks guidance when needed.
  • Ability to make decisions based on education and or clinical experience, which reflect consistency with Center's Guiding Principles, Policies and Procedures, and Standards of Care.
  • Understanding and practice of Trauma Informed Care principles.
  • Wraparound Care Coordinators must possess a minimum of B.A or B.S. degree in social work, psychology or related field with extensive experience in human services. Experience can be substituted for education.
  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.
  • Effective verbal and written communication skills.
  • Strong interpersonal skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Ability to analyze complex information, and to define and solve problems.
  • Ability to work effectively in a team environment.

 Ability to display regular and reliable (in-person, if required) attendance.

  Understanding and practice of Trauma Informed Care principles.

Essential Duties

The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.

1.Attends and participates in all scheduled team meetings.

o  Staff will attend meetings as scheduled and sign in for meetings to acknowledge their presence and understanding of the topics discussed.

o  If a meeting is missed, staff will get with supervisor within one week to review the information.

2.  Provides courteous, professional services at all times.

o  Maintains professional boundaries.

o  Interacts with center staff, individuals, and community members in a professional manner.

3.  Carries a Mental Health caseload for Denton County.

o  Intensive Case management (10 max or more when permitted by HHSC)

4.  Ensures each individual served are provided services as required by the level of care assignment for each month.

o  Engagement measure with consumers is at 65.6% on a monthly basis.

o  Maintains contact standards as defined in Texas Resilience and Recovery (TRR) guidelines.

o  Organizes and facilitates monthly meetings with families and their support system.

5.  Administers clinical assessments and paperwork to determine appropriateness of level of care assignments required to meet individual's needs.

o  Required paperwork includes: CANS, Wrap Plans, Recovery Plan, Substance Abuse Needs Inventory, Rights Review, Family Story, Need Statements, and consent forms.

6.  Is familiar with all applicable Texas Administrative Code (TAC), fidelity tools and Texas Human Health Services (HHSC) contract information and Center policies and procedures.

o  All documentation is completed within two business days, to meet or exceed 95% monthly.

7.   Ensures individuals charts are in compliance with HHSC contract requirements and standards.

o  Responds to all chart audits by assigned deadline.

o  All paperwork will be signed in black ink with credentials and date.

o  Provides emergency/crisis intervention when necessary according to HHSC and Center policies and procedures.

8.  Completes other duties as assigned.

o  All assignments given are completed in a timely manner.

o  Has the ability to work independently and make decisions but seeks guidance when needed.

9.  Assists individuals served during a crisis episode by ensuring they are linked to appropriate Center programs (MCOT, Psych Triage, Crisis Prevention Liaison, etc.).

10.  Participates in the Center's Zero Suicide Initiative.

o  Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention.

o  Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation.

o  Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed.

Monday through Friday:8am-5pm

Pay Range: $21/hr
