Child and Adolescent Wraparound Team Lead

General Description

Under the direction of the Program Manager of Child and Adolescent services and within federal, state and local guidelines provides support to Wraparound Facilitators and performs routine and intensive case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. This is a grant funded position.

The Wraparound/Yes Waiver Team Lead is responsible for supporting Wraparound and Yes Waiver staff in training and adherence to program guidelines to ensure program fidelity. Working with families of children/adolescents with various mental health diagnoses and serving a criminal sentence within the community (probation) by facilitating the Wrap Around planning process, which is family centered, community-oriented, strengths-based and highly individualized. The Wrap Around planning process addresses unmet needs both within and outside of the family’s home and formal human service systems.

Other responsibilities of the Wraparound Team Lead are to monitor and coordinate YES Waiver service provisions and service providers to individuals participating in the YES Waiver Program. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Education, Training and Experience (including licensure and certification)

  • Graduation from an accredited four-year college or university with major course work in a human service field to qualify as a Qualified Mental Health Professional (QMHP), is required.
  • Completion of state Wraparound trainings, including Introduction to Wraparound; Engagement in Wraparound, and Intermediate Wraparound, and Advanced Wraparound Supervisor training within the first year of hire, is required.
  • Minimum of one year related mental health experience is required.
    • Experience may include participation as a member of an Interdisciplinary Treatment Team in developing and implementing individualized program plans for consumers receiving mental health services.
  • Experience working directly with children and/or adolescents and their families for one year is required.
  • Wraparound/Yes Waiver experience is preferred.
  • Ability to collaborate closely with a team of family members and professionals to develop and implement individualized plans of care.
  • Ability to adapt to flexible schedule to meet with the family teams at times that are convenient to the team, to promote active participation of all wraparound team members.
  • Supervisor experience preferred.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

  • Possession of a valid driver’s license and current automobile insurance.
  • Individuals with an out of state driver’s license must be able to obtain a driver’s license in the state of Texas within thirty (30) days.
  • Successful completion of all position-specific training within thirty (30) days of employment is required.
  • Completion of state Wraparound trainings, including Introduction to Wraparound; Engagement in Wraparound, and Intermediate Wraparound, and Advanced Wraparound Supervisor training within the first year of hire, is required.
  • Individual needs to meet the requirements to qualify as a QMHP (Qualified Mental Health Professional).
  • The position requires adherence to all applicable laws, regulations, policies, procedures and/or guidelines of HHSC and DCMHMR. Other specific laws, regulations, policies, procedures and/or guidelines which must be followed including Texas Administrative Code (TAC), Case Management Guidelines/UM Guidelines, NWIC (National Wraparound Implementation Center) and Yes Waiver program guidelines.

Knowledge, Skills, and Abilities

  • Knowledge of
    • Knowledge of community resources, of case management delivery systems, and of program policies and procedures.
  • Local, state, and federal laws related to the program areas.
  • Community mental health services.
  • Serious and persistent mental disorders, personality disorders, and substance use syndromes encompassed in the DSM-V; as well as knowledge of common psychoactive medications used to treat neurobiological diseases of the brain.

  • Operating a computer and applicable software and systems (Word, Excel, OneNote, EHR).

  • Make decisions based on education and or clinical experience, which reflect consistency with Center’s Guiding Principles, Policies and Procedures, and Standards of Care.
  • Maintain a flexible schedule to meet the needs of individuals served as well as supervision of team members.

Essential Duties

Below is a list of essential duties of the position. Other duties may be assigned as needed.

  • Attends and participates in all scheduled team meetings.
    • Staff will attend meetings as scheduled and sign-in for meetings to acknowledge their presence and understanding of the topics discussed.
    • If a meeting is missed, staff will get with supervisor within one week to review the information.
    • Attends monthly Program calls with Yes Waiver Liaison and NWIC Coach.
  • Provides courteous, professional services at all times.
  • Maintains professional boundaries.
  • Interacts with center staff, individuals, and community members in a professional manner.
  • When a caseload is in place, the team lead will meet with 65% of caseload monthly to ensure meeting program service provisions.
    • Provides written updates to all prescribers meeting with individuals served in program.
  • Carries a Mental Health caseload for Wraparound and Yes Waiver when needed. This position caseload should remain at 5 or below, where possible.
  • Ensures each individual served are provided services as required by the level of care (LOC) assignment for each month.
    • Maintains contact standards as defined in Texas Resilience and Recovery (TRR) guidelines.
  • Administers clinical assessments and paperwork to determine appropriate level of care required to meet individual’s needs.
    • Required paperwork includes Child and Adolescent Needs and Strengths Assessment (CANS), Wrap Plans, Recovery Plan, Substance Abuse Needs Inventory, MH Consent to services, consent forms and other necessary paperwork as needed.
  • Is familiar with all applicable TAC, fidelity tools, HHSC contract information, and Center policies and procedures.
    • All documentation is completed within two business days, to meet or exceed 95% monthly.
    • MHC CM Intensive notes include fidelity words.
  • Ensures individuals charts are in compliance with HHSC contract requirements and standards.
    • Responds to all chart audits by assigned deadline.
    • All paperwork will be signed in black ink with credentials and date.
    • Completes 1 Wrap Facilitator/Yes Waiver chart audits per month, per staff.
  • Provides emergency/crisis intervention when necessary, according to HHSC and Center policies and procedures.
  • Backup in absence of Program Manager of Child & Adolescent services to maintain and monitor Yes Waiver Inquiry list and phone line.
  • Trains staff in effective service provision, caseload management, and documentation requirements. Educating staff on contracts and fidelity requirements specific to YES Waiver and Wraparound programs.
  • Productivity for this position is determined by both clinical services, documented supervision and on-going training and development with team members.
  • Collaborate with community partners, agencies, and stakeholders to ensure seamless service delivery and access to resources and support. Quarterly community outreach will be completed and documented.
  • Assists in completing YES Waiver TA Report to Yes Waiver Liaison monthly, due by 9th of each month. Staff will provide the completed report to Program Manager of Child & Adolescent Services by the 6th of each month, for their review and submission.
  • Assists in completing Yes Waiver Quarterly Report by the 15th of the month for each quarter due. Staff will provide the completed report to Program Manager of Child & Adolescent Services by the 10th of each month, for their review and submission.
  • Effectively monitors the following and provides information and concerns to PM of Child & Adolescent Services, for additional direction where needed:
    • Create Team WFH Schedule
    • Review Tracking Sheet
    • Review 1 Wraparound Plans monthly and provide feedback to staff and manager by the 15th of the month.
    • Monthly Group Supervision with each Yes Waiver and/or Wraparound Facilitator staff.
    • Monthly observation and supervision with each staff member.
    • Monthly Team Meetings- completes agenda and follow up notes to staff after meeting completion.
    • Run MEV (Medicaid) to ensure there is timely follow up on those with ‘Pending’ Medicaid status, in the absence of the Manager of Child and Adolescent Services or as needed.
    • Update CE Renewal Board as needed to ensure staff and individuals and families served are aware of CE ending.
  • Monitor staff caseloads and report concerns or successes to Program Manager.
  • Completes other duties as assigned.
    • All assignments given are completed in a timely manner.
    • Has the ability to work independently and make decisions but seeks guidance as needed.
