Under the direction of the Program Manager of Mobile Crisis Outreach Team (MCOT) Services and within federal, state and local guidelines performs moderately complex case management work.
This position is responsible for ensuring all individuals who complete a Risk of Harm (ROH) assessment receive a 24hr follow up appointment, within 24 hours of their initial assessment, and connect individuals to services within the Denton County MHMR or in the community. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Education, Training, and Experience
- Graduation from an accredited four-year college or university with major course work in a human service field as required to qualify as a Qualified Mental Health Professional (QMHP) required.
- Previous Crisis experience and 6 months experience in mental health field preferred.
- English/Spanish bilingual preferred.
Registration, Certification, Licensure, and Other Qualifications
- Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
- Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.
- Successful completion of all position-specific training within thirty (30) days of employment is required.
- Must possess and maintain a valid driver's license and automobile insurance.

- Skills in interviewing, listening, and providing case management services. Ability to triage and determine need for further assessment, provide appropriate referrals/resources to clients, prepare reports, and communicate effectively.

- Computer skills required, specifically use of Microsoft Office, Microsoft Outlook, and the Center's electronic health record system.

Essential Duties
The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.
- Attends and participates in all scheduled meetings.
- Staff will attend all Crisis team meetings as scheduled and sign in for meetings to acknowledge their presence and understanding of the topics discussed.
- If a meeting is missed, staff will get with supervisor within one week to review the information.
- Provides courteous, professional services at all times.
- Maintains professional boundaries.
- Interacts with center staff, clients, and community members in a professional manner.
- Enters time worked accurately each day and submits time sheet by deadline given.
- Provides emergency/crisis intervention when necessary according to HHSC, trauma informed care principles, and center policies and procedures.
- Completes triages with individuals who present with risk of harm to self or other and determines need for MCOT QMHP/LPHA intervention.
- Completes safety plans with individuals as needed.
- Assists with safety monitoring as needed for individuals experiencing a crisis.
- Works collaboratively with MCOT members to ensure all necessary level of care (LOC) 9,0, and 5 services are provided.
- Completes follow up, LOC5 appointments, and case management appointments as assigned within deadline given.
- Communicates recommendation to necessary staff (i.e. Counselor, Supervisors, Psychiatrist, Case Manager, etc.…)
- Develops Recovery plans collaboratively with each individual.
- Recovery plans are completed during LOC5 appointment, case management appointment, or at time of modified intake into Mental Health services.
- All persons with a substance abuse diagnosis, have a Recovery Plan goal which addresses substance abuse needs.
- All Recovery Plan objectives are individualized and measurable.
- The Recovery Plan is reviewed as needed and updated when client identifies new goals or completes goals.
- Effectively managers the LOC0 case load to ensure all individuals who receive ROH assessment receive follow up as necessary.
- Completes 24hr follow up appointments as assigned.
- Schedule and completes 2nd follow up appointment for all LOC0 clients and further follow ups as needed.
- Completes LOC5 appointments as needed.
- Maintains and updates online schedule daily.
- Effectively manages a caseload of 15-30 clients between COPC (Denton) and FMOC (Flower Mound) locations; if applicable.
- Ensures each client on LOC5 caseload is seen face to face a minimum of once per week and completes documentation to include presence or absence of suicidal and homicidal ideations, progress/lack of progress towards recovery goals, resources provided, and next appointment date and time.
- Completes transition form (within 4 weeks of discharge), discharge Child and Adolescent Needs and Strengths Assessment (CANS)/Adult Needs and Strengths Assessment (ANSA), and discharge summary within a week of discharge of individual.
- Completes modified intakes as needed.
- Provides skills training in a collaborative manner with individuals as needed.
- Provides referrals and ensures individual is linked to community resources or Denton County MHMR Mental Health services for continued care.
- Cancels remaining appointments as needed and notifies program manager of need for chart audit.
- Makes community-based visits as needed for individual contact.
- Ensure weekly face to face contact is made with individuals on caseload.
- Completes unscheduled home visit, sends letters, completes phone follow ups for any individuals who do not attend scheduled appointments.
- Staffs with supervisor and completes welfare check on individuals who present with significant risk.
- Documents all services provided to a client or on behalf of a client according to center guidelines.
- Documentation is completed within 2 business days and uploaded into Smart Care if applicable.
- Corrections are completed within 2 business days of being notified.
- Responds to emails within a timely manner.
- Contact supervisor or designated backup for clarifications of procedures or clinical assistance as needed.
- Staff communicates urgent clinical and logistical needs with appropriate manager.
- Contacts Team Lead or Program Manager for clarification of protocols or needed direction or assistance.
- Communicates need for assistance with any follow ups as needed.
- Shares knowledge and resources by collaborating with teammates and sharing community resources when beneficial to client's treatment.
- Is familiar with all applicable TAC, fidelity tools and HHSC contract information and Center policies and procedures.
- All documentation is completed within two business days.
- Skills training notes include fidelity words.
- All documentation is complete, accurate, and compliant with Center and HHSC guidelines.
- Signs company cars out prior to use and in after use, completes travel log in each car, completes maintenance request upon identifying problems, fills cars up with gas using center gas card when cars reach ¼ tank, and is follows center policy regarding accidents in center vehicles or on center time.
- Completes tasks in a manner that fosters principle of creating a safe place for both staff and clients.
- Identify and report safety hazards to the appropriate manager.
- Complete Incident Reports according to Denton County MHMR Policies and Procedures.
- Ensures individual's charts are in compliance with HHSC contract requirements and standards.
- Responds to all chart audits by assigned deadline.
- All paperwork will be signed in black ink with credentials and date.
- Works in a team setting and provides support and assistance to other MCOT members as needed.
- All assignments given are completed in a timely manner.
- Has the ability to work independently and make decisions but seeks guidance when needed.
- Participates in the Center's Zero Suicide Initiative.
Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention.
Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation.
Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed.

Pay Range- $21-$23/hr
Schedule- Monday-Friday 8am-5pm
