Director of Nursing

Under the direction of the Chief Medical Officer and within federal, state, and local guidelines, performs advanced (senior-level) nursing work. Work involves planning, developing, coordinating, and evaluating nursing activities and providing for the care and treatment of individual served. Work may include serving as a resource on nursing practices and education programs by performing consultative and administrative work. Assigns and supervises the work of others. 

The Director of Nursing (DON) is responsible for the administration and evaluation of the nursing services provided through Denton County MHMR Center. The DON determines nursing service requirements while providing for the delegation of nursing care and treatment of clients in the Assertive Community Treatment team (ACT), Mobile Crisis Outreach Team Crisis Teams (MCOT), Crisis Residential Unit (CRU), Psychiatric Triage, The Center for Integrated Health (TCIH) and other MH Units of Denton County MHMR Centers. The coordination and supervision of the nursing staff and nursing related clinic operations of the Mental Health Clinic Outpatient and Medication Services are also under the direction of this individual. This position will work collaboratively with Chief Medical Officer and the Director of Clinic Operations to ensure the operations of the clinic are running smoothly and efficiently. This position requires management experience as well as in depth clinical experience with individuals with chronic mental illness. The ability to quickly prioritize needs and to coordinate services based on these needs is also a must. This position ensures that direct nursing care in all programs is provided in compliance with the principles and standards of Health and Human Services Commission (HHSC), and the Texas Board of Nurse Examiners rules and regulations. Local, state, and federal statutes/laws applicable to service delivery also apply. The DON participates in planning, developing, implementing, and monitoring program operations, policies, and procedures to ensure efficiency of the MH department as well as the MH outpatient clinics. Attends meetings and gives input while serving as a member of the UM committee. Works under minimal supervision, with extensive latitude for the use of initiative and independent judgment.

Education, Training and Experience (including licensure and certification)

Bachelor's degree of Science (BSN) from four-year college or university; requires two-year supervisory experience. Requires one-year experience with individuals that have a mental illness diagnosis. Requires one-year experience with those individuals that have developmental disabilities. Must have current RN license with Board of Nurse Examiners in the state of Texas and be in good standing. Experience in developing policies and procedures, infection control, and managing budgets preferred.

Registration, Certification, Licensure, and Other Qualifications

  • Must maintain professional nursing licensure with the state of Texas.
  • Complete all required CNE’s as required by the Board of Nurse Examiner’s.
  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver’s license and automobile insurance.
  • Individuals with an out-of-state driver’s license must be able to obtain a driver’s license in the state of Texas within thirty (30) days.
  • Successful completion of all position-specific training within thirty (30) days of employment is required.

Essential Duties include the following.  Other duties may be assigned.

1.  Provide administrative oversight and direction of the MH nursing department and MH outpatient clinics. Provides clinical supervision and delegation of training to all nursing and clinic support staff serving the mental health outpatient clinics nursing staff, TCIH, Psych Triage nursing staff, crisis residential nursing staff, ACT nursing staff and MCOT crisis nursing staff.

2.  Over-see's the clinical training for the Program Clinic Assistants (PCA's). Assign, train and provide clinical oversight for the Lead PCA's and PCA's in the performance of their clinical job duties as outlined in the job description. Perform RN Delegation, training, oversight and evaluation for nurses in clinics, psychiatric triage, and integrated clinic and in the crisis residential facility for any item that requires RN Delegation.

3.  Participate in Prescription Assistance Program (PAP) team meetings. Assign tasks as needed to nursing staff following each meeting. Participate in FRAQUM monthly UM meetings. Prepare and deliver reports in the FRAQUM meeting on a regular or as needed basis.

4.  Create the agenda and run the nursing team meetings on a weekly basis. Maintain minutes of all nursing meetings. Participate in quarterly meetings with the contract pharmacy. Participate in scheduling decisions in a collaborative fashion with the Clinic Operations Manager as needed.

5.  Serve in and manage the nurse in charge, nurse on call and evaluation phone rotations, adhere to unit procedures for specific duties of each task. Develop rotation and ensure all nursing staff are aware of rotations. Provide and update the Patient and Family Education (PFEP) material for Center Qualified Mental Health Professionals and Licensed Professionals of the Healing Arts according to contract requirements.

6.  Develops medication education and training curriculum for staff and consumers (including self-medication and signs and symptoms of illness on a scheduled basis). Trains consumers and staff in medication education curriculum. Provides medication education and training sessions as scheduled with consumers.

7.  Assess individuals on an as needed basis for general health status, mental health status, and response to, as well as understanding of medications and treatments that are recommended. Uses skilled professional medical knowledge to identify medically at-risk individuals and persuade recipients or potential recipients to enter care through the Medicaid system. Refers, coordinates and/or monitors the delivery of medical services. May also be involved in program planning and policy development including interagency coordination.

8.  Provides direct nursing services and education to clients within the scope of the Nurse Practice Act and the policies, procedures, and guidelines of DCMHMR. Education involves understanding illness, managing signs and symptoms of illness, medication education and any other area pertinent to maintaining psychiatric or medical stability. Provides comprehensive nursing assessment as outlined in the Scope and Standards of Psychiatric-Mental Health Nursing Practice.

9.  Provides direct and telephonic services for individuals being served in the crisis program or active clients that are accessing the nurse on call for crisis medication management. Triages all situations as reported and coordinates services as clinically appropriate. This triage and coordination include the effective use of psychiatric emergency and crisis evaluations in the MH outpatient clinics.

10.  Attends UM meetings to assess the necessity for and the adequate provision of medical care and services for individuals in Denton County MMHMR services. Works in consultation with all program areas and outpatient clinics to ensure that client's needs are met in each of these programs. Acts as a liaison of the medical aspects of the program with providers of service and other agencies that provide medical care.

11.  Reviews medical records, client assessments, and other relevant documents to determine compliance with facility policies and procedures, Nursing Practice Act, as well as DSHS and DADS requirements. Participates in medical and preliminary clinical death review or independent professional review team activities as needed.

12.  Documents professional nursing activities as per center documentation policy of service provided. Completes all documentation required and in compliance with DCMHMR policies and procedures, Medicaid, Medicare, and any other third-party payer source.

13.  Coordinates services to ensure that third party payment is maximized. Analyzes the provision of services for accuracy and effectiveness. Collects data and completes reports regarding services provided and the outcome of these services as requested by the Chief Executive Officer. Works with center's QM department and provides necessary reports.

14.  Responsible for the development of the outpatient clinic budget in conjunction with Chief Executive Officer. Monitors budget information to ensure efficient unit operations. Gives input to the Child and Adolescent, Crisis and Nursing Budget and other Center program in reference to items needed for nursing and medical requirements.

15.  Administrates and evaluates the New Generation Medication budget and targets. Consistently meets all targets in compliance with state requirements. Works in cooperation with employees, supervisors, and community service providers in the provision of services.

16.  Serve as the Infection Control Nurse for all Center operations. Reviews Infection Control Policy and Procedure and revises as needed. Trains all Center staff in universal precautions regarding Infection Control. Responsible for reporting reportable health issues as required to the Health Department or other governing agency.

17.  Completes supervisory responsibilities in a timely manner

  • Interviews and hires new employees per NEO-GOV requirements.
  • Determines hours of work for employees.
  • Directs, supervises, recognizes and coaches employees.
  • Plans work operations.
  • ?Determines how work will be performed.

18.  Evaluates job performance of employees. Assist other Administrators in completing performance evaluations for nurses in other programs such as ACT, crisis residential, integrated and psychiatric triage nurses.

  • Takes corrective action with employees in a timely manner.
  • Monitors and maintains employee staff development and training for assigned employees.
  • Supervisor will address performance issues and ensure that the staff is operating within acceptable Center guidelines.

19.  Completes any additional duties as assigned by Chief Executive Officer and Medical Director.

20.  Perform additional duties as deemed necessary by department supervisor within designated timeframes.

21.  Positions are posted and filled in compliance with Center procedures and timelines.

22.  Supervisor will provide individual written supervision to the staff supervised by the position, as needed. Supervision will provide guidance for outstanding needs of staff and contractors. (i.e. Review documentation for accuracy and content or provide input) Supervisor will effectively communicate with staff the information that is needed for supervisors to effectively manage their teams and programs.

23.  Performance Evaluations must be completed within required timeframes and provide accurate depiction on employee's performance and outline goals for improvement if needed.

24.  Ensures employees comply with education and training requirements, 100% compliance.

25.  Ensures that employees are trained to perform their jobs and that various requirements of the position are communicated to the employee.
