HCS Service Coordinator

Under direction of the Program Manager of IDD Authority Services and within federal, state, and local guidelines, performs routine work meeting the needs of individuals with developmental disabilities. Work involves assessing and determining individual needs, developing, and monitoring of Person Directed Plans, and connecting individuals with services and supports.

The Service Coordinator will provide Service Coordination to individuals with Intellectual and Developmental Disabilities (IDD). The position maintains a varied caseload of persons with multiple, complex needs. Job responsibilities are conducted in accordance with the Texas Administrative Code (TAC), Department of Aging and Disability Services (DADS) and DADS program specific handbooks, Performance Contract, Medicaid requirements, Center policies and procedures as well as other applicable state and federal laws. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Education, Training and Experience (including licensure and certification)

This position requires a bachelor's degree from an accredited college or university with a major in social, behavioral, or human service field. A minimum of one year's experience in the human services field is preferred. MHMR or Service Coordination experience is preferred.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

· All job specific training must be completed within 30 days of hire.

· Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.

Knowledge, Skills, and Abilities

· Knowledge of rehabilitation programs for individuals with developmental disabilities.

· Skill in the operation of computers and in applicable computer software.

· Ability to collect and analyze data, to design therapeutic programs, to prepare and maintain records, and to communicate effectively.

· Ability to independently manage time and schedule meetings, with strong organizational skills.

Essential Duties

· Completes Service Coordination Assessments on all individuals as regulated by the TAC and Center procedures, document Service Coordination Level of Need in the Person Directed Plan (PDP) and maintain level of contact with individual as assessed. Completes DADS Form 8647 on all clients and annually thereafter.

· Develops and revises the PDP for each individual in a timely manner, as required by the TAC and Center procedures.

· Holds initial and annual PDP meetings and develops those PDP's. Revises the PDP throughout the plan year when changes occur.

· Monitors the PDP and implementation of personal outcomes, services and supports including services through various funding sources.

· Serves as an advocate for individuals by reviewing rights annually and as needed, informing individuals/LARs of the Center's complaint process and promoting individual choice.

· Reviewing Rights information orally and in writing annually, or as requested/needed. Advocating, coordinating, and mediating when situations arise.

· Maintains positive relationships with individuals, family members, service providers, Center employees, other agencies and the community. Returns phone calls and e-mails in a timely manner. Follows up with individuals and/or issues in a timely manner.

· Completes all required documentation as required and in accordance with Center policies and procedures, DADS contract, Medicaid requirements, the TAC and the Service Coordination TAC.

· Completes program specific PDP's and forms. Completes progress notes within three business days.

· Responds to crisis intervention with minimal supervision. Responds to individual crisis situations when they arise, ensuring the health and safety of the client, and making appropriate reports to entities within the Center's designated timeframes.

· Provides back-up to other service coordinators during absences or vacancies.

· Carries a varied, temporary, caseload during program vacancies as requested.

· Provide on-going service coordination and serve as a liaison between individuals, families, and service providers.

· Meets with clients on at least a monthly basis, or more frequently, if necessary, to provide ongoing monitoring and assistance to the client and family.

· Make referrals for consumers as needed in accordance with service plan and consumer needs. Completes referrals within two weeks of the need, service, and/or support being identified.

· Works both independently and as part of a team.

· Independently schedules and makes client contacts.

· Participates as a member of the clients Service Planning Team.

Documentation & Compliance:

· Required monthly documentation of face-to-face contacts with additional collateral contacts based on caseload assignment.

· Documentation of face-to-face monthly contacts and 3 additional collateral contacts for all Medicaid clients.

· Documentation of at least 3 attempts to contact a client if a monthly face-to-face visit does not occur, with at least one of the attempted contacts before the 15th of the month.

· Complete waiver program enrollments. Completes assigned HCS/TXHML enrollments in the designated timeframes as regulated by DADS.

Performs additional duties as deemed necessary by department supervisor within designated timeframes. Completes additional duties as assigned.

General Revenue (GR)

· Refer individuals for requested services community based and agency based.

· Authorize services to be provided through General Revenue Funding.

· Ensure services requested and authorized are being utilized.

· Assist with request for alternative placement.

· Provide services to individuals in their own homes and/or their family's home.

Texas Home Living (TxHmL)

· Completes ID/RC Assessments, Individual Plan of Care, and ICAPs within designated time frame.

· Performs responsibilities regarding transfers, suspensions, or terminations in a timely manner.

· Performs utilization review to monitor delivery of services.

· Abide by TxHmL TAC and Handbook.

· Assist with request for alternative placement.

· Provide services to individuals in their own homes and/or their family's home.

Home and Community Based Services (HCS)

· Reviews ID/RC Assessments and Individual Plan of Care within designated time frame.

· Performs responsibilities regarding transfers, suspensions or terminations in a timely manner.

· Performs utilization review to monitor delivery of services.

· Abide by HCS TAC and Handbook
