IDD Service Coordinator

Under direction of the PM OF SERVICE COORDINATION AND GUARDIANSHIP and within federal, state, and local guidelines performs routine work meeting the needs of individuals with developmental disabilities.  Work involves assessing and determining individual needs, developing, and monitoring of Person Directed Plans, and connecting individuals with services and supports.

The Service Coordinator will provide Service Coordination to individuals with Intellectual and Developmental Disabilities (IDD). The position maintains a varied caseload of people with multiple, complex needs. Job responsibilities are conducted in accordance with the Texas Administrative Code (TAC), HHSC Performance Contract, HHSC program specific Handbooks, Medicaid requirements, Center policies and procedures as well as other applicable state and federal laws. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

This position can work partially remote once all training is complete and productivity measures are consistently met.

Education, Training, and Experience

  • This position requires a bachelor's degree from an accredited college or university with a major in social, behavioral, or human service field.
  • At least one-year human services experience is preferred. LIDDA or Service Coordination experience is preferred. 

Registration, Certification, Licensure, and Other Qualifications

  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver's license and automobile insurance.

o  Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.

  • Successful completion of all position-specific training within thirty (30) days of employment is required.

Knowledge, Skills, and Abilities

  •   Knowledge of programs for individuals with Intellectual and Developmental Disabilities and community resources
  •   Skill in the operation of computers and in applicable computer software.
  •   Ability to independently organize and prioritize a variety of assignments while managing time effectively.
  •  Skilled in professional, effective communication, both orally and in writing.
  •   Desire to monitor and advocate for individuals in services.
  •   Skilled in effective problem solving and conflict resolution.
  •   Ability to work in a non-judgmental manner with individuals whose values and belief systems may be different from the employee.
  •  Ability to display regular and reliable (in-person, if required) attendance.
  • Understanding and practice of Trauma Informed Care principles.

Essential Duties

The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.

  1.   Provides on-going services to assist individuals with Intellectual and Developmental Disabilities, or related conditions, in developing a Person Directed Plan (PDP) that identifies desires outcomes and services.
  2. Meets with clients on at least a monthly basis, or more frequently, if necessary, to provide ongoing monitoring and assistance to the client and family.
  3. Documentation of face-to-face monthly contacts and 3 additional collateral contacts for all Medicaid clients.
  4. Completes progress notes within three business days.
  5. Holds initial and annual PDP meetings. Revises the PDP throughout the plan year when changes occur.
  6. At least quarterly, documents a review of progress towards achieving desired outcomes in the PDP and documents delivery and satisfaction with each identified service.
  7. Serves as advocate for individuals by reviewing rights annually and as needed, informing individuals/LARs of the Center's complaint process and promoting individual choice. 
  8. Maintains positive relationships with individuals, family members, service providers, Center employees, other agencies and the community. Returns phone calls and e-mails in a timely manner. Follows up with individuals and/or issues in a timely manner.
  9. Completes all required documentation as required and in accordance with Center policies and procedures, HHSC contract, Medicaid requirements, applicable program handbooks, and the TAC.
  10. Responds to individual crisis situations when they arise, ensuring the health and safety of the client, and making appropriate reports to entities within the centers designated timeframes with minimal supervision
  11. Provides back up to other service coordinators during absences or vacancies. Carries a varied, temporary, caseload during program vacancies as requested.
  12. Works both independently and as part of a team.
  13. Independently schedules and makes client contacts.
  14. Completes assigned HCS/TXHML enrollments in the designated timeframes as regulated by HHSC.
  15. Perform additional duties as deemed necessary by department supervisor within designated timeframes.
  16. Ensures that all required documentation is in each individual's medical record chart.

General Revenue (GR)

  • Refer individuals for requested services community based and agency based.
  • Authorize services to be provided through General Revenue Funding.
  • Ensure services requested and authorized are being utilized.
  • Assist with request for alternative placement.
  • Provide services to individuals in their own homes and/or their family's home.

Texas Home Living (TxHmL)

  • Completes ID/RC Assessments, Individual Plan of Care, and ICAPs within designated time frame.
  • Performs responsibilities regarding transfers, suspensions, or terminations in a timely manner.
  • Performs utilization review to monitor delivery of services.
  • Abide by TxHmL TAC and Handbook.
  • Assist with request for alternative placement.
  • Provide services to individuals in their own homes and/or their family's home.
  • Utilizes and maintains access to the Texas Medicaid & Health Partnership (TMHP) portal, Client Assignment and Registration System (CARE) database, and any other database as deemed necessary.

Home and Community Based Services (HCS)

  • Reviews ID/RC Assessments and Individual Plan of Care within designated time frame.
  • Performs responsibilities regarding transfers, suspensions or terminations in a timely manner.
  • Performs utilization review to monitor delivery of services.
  • Abide by HCS TAC and Handbook.
  • Provide services to individuals in their own homes, their family's home, host home setting, and group homes.
  • Utilizes and maintains access to the Texas Medicaid & Health Partnership (TMHP) portal, Client Assignment and Registration System (CARE) database, and any other database as deemed necessary.
  •  Participates in the Center's Zero Suicide Initiative.

o  Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention.

o  Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation.

o  Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed.
