Under direction of the LIDDA Director and within federal, state, and local guidelines performs routine work meeting the needs of individuals with developmental disabilities. Work involves assessing and determining individual needs, developing, and monitoring of Person Directed Plans, and connecting individuals with services and supports.
The Service Coordinator Team Lead will provide Service Coordination to individuals with Intellectual and Developmental Disabilities (IDD). The position maintains a varied caseload of persons with multiple, complex needs. The Service Coordinator Team Lead will serve as a mentor to their specific program. Job responsibilities are conducted in accordance with the Texas Administrative Code (TAC), HHSC Performance Contract, HHSC program specific Handbooks, Medicaid requirements, Center policies and procedures as well as other applicable state and federal laws. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
This position has the ability to work partially remote once all training is complete and productivity measures are consistently met.
Education, Training and Experience (including licensure and certification)
This position requires a bachelor's degree from an accredited college or university; or an associate degree in a social, behavioral, human service, or health-related field, including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice; or a high school diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
At least one-year human services experience is preferred. LIDDA or Service Coordination experience is preferred.
REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:
- All job specific training must be completed within 30 days of hire.
- Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.

Essential Duties
- Consults weekly with Service Coordinators and the designated Program Manager or Director in regard to staff and program needs.
- Provides training to new Service Coordinators and ongoing trainings as necessary.
- Tracks data, runs program reports, and performs data entry.
- Monitor and review program specific documentation.
- Reviews Annual PDP's and associated forms to ensure completion.
- Meets with staff to provide accurate depiction of performance and outline goals for improvement if needed.
- Reports to supervisor any identified staff performance issues for follow up.
- Conducts interviews with applicants to ensure that vacant positions are filled in compliance with Center procedures and in a timely manner.
- Provides on-going services to assist individuals with Intellectual and Developmental Disabilities, or related conditions, in developing a Person Directed Plan (PDP) that identifies desires outcomes and services.
- Meets with clients on at least a monthly basis, or more frequently, if necessary, to provide ongoing monitoring and assistance to the client and family.
- Documentation of face-to-face monthly contacts and 3 additional collateral contacts for all Medicaid clients.
- Completes progress notes within three business days.
- Holds initial and annual PDP meetings. Revises the PDP throughout the plan year when changes occur.
- At least quarterly, documents a review of progress towards achieving desired outcomes in the PDP and documents delivery and satisfaction with each identified service.
- Serves as advocate for individuals by reviewing rights annually and as needed, informing individuals/LARs of the Center's complaint process and promoting individual choice.
- Maintains positive relationships with individuals, family members, service providers, Center employees, other agencies and the community. Returns phone calls and e-mails in a timely manner. Follows up with individuals and/or issues in a timely manner.
- Completes all required documentation as required and in accordance with Center policies and procedures, HHSC contract, Medicaid requirements, applicable program handbooks, and the TAC.
- Responds to individual crisis situations when they arise, ensuring the health and safety of the client, and making appropriate reports to entities within the centers designated timeframes with minimal supervision
- Provides back up to other service coordinators during absences or vacancies. Carries a varied, temporary, caseload during program vacancies as requested.
- Works both independently and as part of a team.
- Independently schedules and makes client contacts.
- Completes assigned HCS/TXHML enrollments in the designated timeframes as regulated by HHSC.
- Perform additional duties as deemed necessary by department supervisor within designated timeframes.
- Ensures that all required documentation is in each individual's medical record chart.
Specific Program Duties
General Revenue (GR)
- Refer individuals for requested services community based and agency based.
- Authorize services to be provided through General Revenue Funding.
- Ensure services requested and authorized are being utilized.
- Assist with request for alternative placement.
- Provide services to individuals in their own homes and/or their family's home.
Texas Home Living (TxHmL)
- Completes ID/RC Assessments, Individual Plan of Care, and ICAPs within designated time frame.
- Performs responsibilities regarding transfers, suspensions, or terminations in a timely manner.
- Performs utilization review to monitor delivery of services.
- Abide by TxHmL TAC and Handbook.
- Assist with request for alternative placement.
- Provide services to individuals in their own homes and/or their family's home.
- Utilizes and maintains access to the Texas Medicaid & Health Partnership (TMHP) portal, Client Assignment and Registration System (CARE) database, and any other database as deemed necessary.
Home and Community Based Services (HCS)
- Reviews ID/RC Assessments and Individual Plan of Care within designated time frame.
- Performs responsibilities regarding transfers, suspensions or terminations in a timely manner.
- Performs utilization review to monitor delivery of services.
- Abide by HCS TAC and Handbook.
- Provide services to individuals in their own homes, their family's home, host home setting, and group homes.
- Utilizes and maintains access to the Texas Medicaid & Health Partnership (TMHP) portal, Client Assignment and Registration System (CARE) database, and any other database as deemed necessary.
