General Description

 Under the direction of the Director of TCOOMMI Services and within federal, state and local guidelines provides moderately complex case management work.  Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs.  May train others.

This position will serve individuals involved in the criminal justice system by providing routine case management, psychosocial rehabilitation services, supported employment services, supported housing services, dual diagnosis treatment, and crisis resolution to adults: who are on probation and or parole, who currently or historically have frequent involvement with the criminal justice system, who have evidence of co-occurring psychiatric and substance use disorders (COPSD).  Will be responsible for planning and monitoring services to ensure identified needs are being met. Works under moderate supervision, with limited latitude for the use of initiative and independent judgement.

Education, Training, and Experience

  •   Graduation from an accredited four-year college or university with major course work in a human services field as required to qualify as a Qualified Mental Health Professional (QMHP). 
  •   Six (6) months previous experience working in criminal justice or a related field is required.
  •   Minor course work or double major in Criminal justice and one (1) year prior experience in criminal justice or a related field preferred.

Registration, Certification, Licensure, and Other Qualifications

  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver's license and automobile insurance.

o  Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.

  •   Successful completion of all position-specific training within thirty (30) days of employment is required.

Knowledge, Skills, and Abilities

  •   Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures.
  •   Ability to assess clients' needs, to coordinate clients' services, to communicate effectively, and to train others.
  •   Understanding and practice of Trauma Informed Care principles.

Essential Duties

The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.

1.  Manages a caseload of Level of Care three (3) clients on parole; organizes and tracks the required minimums for each client; ensures these (3.5) hours are met each month; ensures 95% compliance with the ANSA per DSHS contract requirements and community standards.

2.  Provides and documents services in compliance with Medicaid rules and standards established by other payers/funding sources, as demonstrated by successful scores on compliance audits.

3.  Ensures completion of Treatment Plan, ANSA, Review of Rights, Family Needs Assessment, Substance Abuse Inventory and other required documentation prior to expiration/lapse date and explains the program goals and expectations to consumer at first non-crisis face-to-face meeting; completes PFEP and reviews Patient & Family Education material with each consumer prior to their seeing the psychiatrist; provides skills training to consumers to address expressed and implied needs. The frequency of training should mirror what is specified in each consumer's ANSA ? LOC1-3.

4.  Completes all closeouts and discharges in a timely manner; performs QMHP (Qualified Mental Health Professional) duties as assigned by supervisor, maintaining a caseload of 20-25 clients.

5.  Provides verbal and physical PMAB intervention correctly as necessary; provides CPR/First Aid as needed and appropriate until medical professionals arrive or as directed by a licensed medical professional; complies with staff development and is responsible for scheduling and completing DCMHMR required training; attends all meetings, functions, trainings, etc. for TCOOMMI via interoffice trainings and modules or via TCOOMMI/TCA meetings (as funds are available).

6.  Interviews clients, reviews records, and acts as member of the Interdisciplinary Team to evaluate and advocate for effective client treatment, and transition planning; ensures open line of communication exists between DCMHMR and both TDCJ parole division and Community Supervision and Corrections Department (CSCD) at all times; intervenes in crisis situations, problematic, and sensitive issues involving TCOOMMI staff, clients on probation or parole and the parole/CSCD officers; ensures that Parole/CSCD officer is contacted at least once a month for each client assigned to their caseload.

7.  Formulates programs for treatment that consider client needs as expressed by client, family members and service providers; monitors consumers progress toward outcomes set forth in Treatment Plan; monitoring is conducted at the Center or in the consumer's environment, which may include jail, home, work, etc. Monitors MOU (Memorandum of Understanding) expectations with Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI); complies with all aspects of MOU mentioned above including scheduling all TCOOMMI appointments, maintaining communication with Parole officers and reporting compliance of all clients.

8.  Establishes and maintains relationships with other community agencies to build a referral network; participates in team building activities by attending scheduled team meetings, utilizing clinical supervision as needed and maintaining positive communication with colleagues and supervisor.

9.  Participates in the Center's Zero Suicide Initiative.

a.  Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention.

b.  Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation.

c.  Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed.
