Under the direction of the Child and Adolescent Program Manager and within federal, state and local guidelines performs Wraparound/Intensive Case Management (ICM). Work involves developing and maintaining long-term contact with youth and families, service providers for medical, social, educational, and related service needs; and support members who will be on the youth and family's team.
The Wraparound Facilitator should be an empathetic, creative, and resourceful individual. They should be flexible and committed to supporting children with behavioral challenges or various mental health diagnoses. All children and families deserve access to high quality services, and we strive to provide this support to those we partner with. As a Wraparound Facilitator, you will be working collaboratively with families, their support systems, and other professionals or community partners to develop individualized, strength-based plans of care. The Wraparound planning process addresses the reasons why individuals behave certain ways both within and outside of the family's home and formal human service systems.
Education, Training, and Experience
o Bachelor's Degree in Human Services Field as required to qualify as a QMHP.
o Minimum of (1) year related experience is preferred.
o Experience may include participation as a member of an Interdisciplinary Treatment Team in developing and implementing individualized program plans for MH consumers. Must have experience and a commitment to working directly with children and/or adolescents and their families.
o Needs a good knowledge base of mental illness diagnoses and behavior disorders in children and/or adolescents.
o Must be able to collaborate closely with a team of family members and professionals to develop and implement individualized plans of care.
o Must have a flexible schedule in order to meet with the family teams at times that are convenient to the team, so as to ensure active participation of all wraparound team members.
o Must complete State required wraparound training: Introduction to Wraparound; Engagement in Wraparound, and Intermediate Wraparound training within the first year of hire.
Registration, Certification, Licensure, and Other Qualifications
o Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.
? Successful completion of all position-specific training within thirty (30) days of employment is required.
Knowledge, Skills, and Abilities
Essential Duties
The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.
1. Writes Progress Notes on all consumer activity and turns in on a daily basis; keeps daily service logs on all direct care activities to be turned in within 2 business days of contact.
2. Responsible for chart compliance for all consumers served on the caseload.
3. Responsible for monitoring YES Waiver Individualized Plans of Care (IPCs) and ensuring clients have access to authorized services
4. Networks actively with other human service providers in an effort to improve resources and service for the chronically mentally ill, including serving on committees as needed.
5. Teach parents/guardians about a child's symptoms and planned interventions in the plan of care, i.e., anger, aggression, anxiety, fear, etc.
6. Acts as Liaison Worker between Denton County MHMR Center and other DSHS (Department of State Health Services) agencies and state hospitals, documenting this in the consumer's charts. Serve as a liaison with NTSH-WF, NTSH-V & WCY
7. Tracks all new admissions daily on the CARE System
8. Maintains a working relationship with the social workers by on-site visits and phone contact on an on-going basis; visits appropriate hospitals once per month & interview Denton County residents.
9. Participates in the Center's Zero Suicide Initiative.
o Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention.
o Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation.
o Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed.
