Under direction of the Connections Program Coordinator and within federal, state and local guidelines, performs moderately complex life skills services. Work involves providing life skills services for clients with mental impairments.

The Connections Rehab Therapist provides transportation for clients to and from rehabilitation programs, psychosocial skills training and individual and/or small group community support services in an individual's home or the community. Duties include frequent contact with clients, assisting clients with activities of daily living, and developing and managing treatment objectives. Under the supervision of the Connections Program Coordinator, assists with preparation of monthly rental assistance and utility check requests, prepares documents for referrals, intake session, and annual recertification including income verification under HUD guidelines, and assists with other program related tasks as assigned by the Connections Program Coordinator and/or deemed a priority within the HUD grant funding. Works under moderate supervision, with limited latitude for the use of initiative and independent judgement.

Education, Training and Experience (including licensure and certification)

Bachelor's degree in a human services field or related field, and one year experience in mental health or intellectual and developmental disabilities. Previous case management experience preferred.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

• All job specific training must be completed within 30 days of hire.
• Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.

Knowledge, Skills and Abilities

·Proficient skills with computers and applicable computer software. Ability to collect and analyze data, to prepare and maintain records, and to communicate effectively. Ability to read and interpret documents i.e. HUD regulations and rules, and procedure manuals. Ability to assess clients' needs, to coordinate clients' services, and to monitor program effectiveness. Ability to maintain a calm and efficient demeanor during crisis situations.

·Moderate mathematics skills. Proficient reading, writing, interpersonal relations and communicative skills; ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write effective case management notes. Ability to effectively present information in one-on-one and small group situations. Highly effective in working objectively with a diverse group of people; must demonstrate strong organizational skills. Knowledge of therapeutic methods and techniques in the rehabilitative therapy area of specialization and of mental illness and mental disabilities.

·Ability to provide life skills services for clients, to prepare concise reports and case documentation, ability to represent DC MHMR and the Connections program at community functions or events, and to train others.

·Understanding and practice of Trauma Informed Care principles.

Essential Duties include the following. Other duties may be assigned.

1.Provides safe and reliable transportation for clients to and from Rehab programs; provides community support services to assigned clients within homeless community; makes face to face contact with clients a minimum of once a month; provides support and skills training to assigned clients that are individualized, supportive, and beneficial for client to overcome daily living problems; initiates and/or coordinates appropriate resources/services to meet the needs of the client.

2.Follows and assists in development of comprehensive service plan; assists clients in developing goals and objectives which recognize individual needs, utilizes personal strengths, natural support resources, and community service agencies; ensures treatment objectives are meaningful to client, measurable, and attainable; ensures that the comprehensive service plan is implemented as written; provides ongoing review of treatment objectives with client.

3.Serves as an active member of unit and Connections Team; participates in problem solving, crisis management, and other team activities as directed.

4.Maintains all documentation and follows review procedures in accordance with grant requirements, agency policy and procedures and billing requirements; documents all contacts with or on behalf of the client in accordance to program policy and HUD requirements; ensures client caseload is reviewed and updated quarterly; ensures secondary records are maintained as outlined by program policy.

5.Maintains all required training; completes all staff development training and HUD training as required for position; updates training as specified under agency procedures or as directed by supervisor; acts as a liaison with all community providers in a professional manner and is a representative of the Center in all communications in the community.

6.Maintains acceptable driving record as required for DCMHMR insurance; performs additional duties as deemed necessary by department supervisor within designated timeframes.
