First Episode Psychosis Skills Trainer - Secondary

General Description

Under the direction of the FEP Team Lead and within federal, state, and local guidelines, the Temporary PRN FEP Case Manager provides moderately complex case management services to individuals between the ages of 15 and 30 experiencing their first episode of psychosis. This is a temporary, as-needed (PRN) position intended to provide coverage based on program needs. FEP is a grant-funded Coordinated Specialty Care program through the Texas Health and Human Services Commission (HHSC). This program uses a team-based, person-centered, and recovery-oriented approach to help individuals manage not only their psychiatric symptoms, but also achieve success in other areas of life, such as education and employment. The FEP program serves Denton County residents between the ages of 15 and 30 who have experienced the onset of psychosis symptoms within the past two years. The FEP team maintains a caseload of approximately 20-30 individuals served, all of whom are supported by an interdisciplinary team. FEP Case Manager duties include developing and maintaining long-term relationships with individuals served, their families, and service providers to address medical, social, substance use, and related service needs. This position may also provide training to others.

Education, Training, and Experience

·Graduation from an accredited four-year college or university with major course work in a human services field as required to qualify as a Qualified Mental health Professional (QMHP), which includes a minimum of 31 college hours in human services courses

·Two (2) years prior experience in a mental health field required and Crisis experience or experience working with individuals who have experienced symptoms of psychosis preferred

Registration, Certification, Licensure, and Other Qualifications

  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver's license and automobile insurance.

oIndividuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.

  • Successful completion of all position-specific training within thirty (30) days of employment and ongoing compliance with annual training requirements is required.

Knowledge, Skills, and Abilities

·Understanding and practicing Trauma Informed Care principles

·Moderate mathematics skills. Proficient reading, writing, interpersonal relations and communicative skills; ability to read and comprehend simple instructions, short correspondence, and memos.

·Skill in identifying measures or indicators of program and staff performance and in operating a computer and applicable software.

·Ability to assess the individuals' needs, to coordinate services, and to communicate effectively.

·Resourcefulness, especially in regard to finding specific resources, forming relationships with community agencies, and helping individuals served access resources

·Knowledge of some of the long-standing community resources; of case management principles, objectives, standards, and methods; and of program policies, and procedures.

·Approachable, non-judgmental, and friendly attitude to aid in forming relationships with clients, families, and resource providers in the community.

·Ability to write effective documentation of services including case management notes, recovery plans, and contact notes. Training will be provided

·Ability to effectively present information in one-on-one and small group situations

·Highly effective in working objectively and a diverse group of people and ability to demonstrate communication, organizational, administrative, and office managerial skills. Computer skills required in both clinical and clerical programs.

Essential Duties

The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.

1.Connect individuals served to and ensuring communication of needs with all other COMPASS Team providers including the Team Lead/Therapist, Supported Employment and Education Specialist, Peer Specialist, Certified Family Partner, Nurse, and Psychiatrist.

2.Works as liaison between the individual served and community providers and advocates for the individual as needed, both in center services and with other community agencies.

3.Meet with clients to identify recovery plan goals and objectives.

4.Follow up with individuals served, on a regular basis, to aid them in achieving their recovery plan goals and objectives. This aid may come in the form of accountability, finding and connecting clients to community resources, education regarding diagnosis and symptom management, and specific skill training.

5.Aid in identifying and alleviating barriers to achieving recovery plan goals and objectives.

6.Provide skills training in a variety of areas including coping skills, activities of daily living, independent living, life management skills, communication, etc....

7.Take extra steps in helping the individuals served connect with community resources, which may include help in filling out applications, attending appointments, and following up to resolve barriers to obtaining resources.

8.Ongoing research and updating additional resources in the community and within the center to build a referral network.

9.Establish and maintain relationships with the families and support system of individuals served.

10.Work collaboratively with treatment team to monitor symptoms and assess for risk on a continuous basis.

11.Refer individuals served to Mobile Crisis Outreach Team when reports of risk reach threshold. Maintain contact and support with individuals served during times of crisis and crisis treatment

12.Provide transportation to the individuals served in the program to and from appointments

13.Aid the individuals served in appropriate discharge planning and connection to the succeeding providers.

14.Participate in team building activities by attending scheduled team meetings, utilizing clinical supervision as needed and maintaining positive communication with colleagues and supervisor.

15.Upkeep various electronic information tracking systems to ensure program fidelity markers are met

16.Duties expected to be done under moderate supervision

17.FEP Case Manager is encouraged to meet individuals served in the community as much as possible and at least once quarterly.

Additionally, all FEP Team members will be responsible for completing the following duties from time to time;

1.Follow up on referrals and complete intake documents for new FEP clients

2.Take turn in answering after-hour phone calls weekday nights and weekends

3.Be a contributing member of the FEP outreach and referral team, which provides periodic community outreach services such as presentations and education.

4.Interact with those individuals served not directly assigned to you, review records, and act as a member of the interdisciplinary team to help evaluate effectiveness of treatment.

5.Lead group skills training sessions for individuals served. Some of which may take place in the evening after 5pm.

6.Provide skills training in areas specific to employment and education to provide coverage for the Supported Employment and Education Specialist.

7.Perform additional duties as deemed necessary by department supervisor within designated time frames.

All Denton County Staff have the following additional duties:

1.Completion of all client documentation via electronic record system within 2 business days of event.

2.Maintaining acceptable driving record as required for the Center's insurance.

3.Training in and adherence to the Trauma Informed Care Practices adopted by Denton County MHMR Center.
