C & A WRAPAROUND FACILITATOR

Under the direction of the Child and Adolescent Program Manager and within federal, state and local guidelines performs Wraparound/Intensive Case Management (ICM). Work involves developing and maintaining long-term contact with youth and families, service providers for medical, social, educational, and related service needs; and support members who will be on the youth and family’s team.

The Wraparound Facilitator should be an empathetic, creative, and resourceful individual. They should be flexible and committed to supporting children with behavioral challenges or various mental health diagnoses. All children and families deserve access to high quality services, and we strive to provide this support to those we partner with. As a Wraparound Facilitator, you will be working collaboratively with families, their support systems, and other professionals or community partners to develop individualized, strength-based plans of care. The Wraparound planning process addresses the reasons why individuals behave certain ways both within and outside of the family’s home and formal human service systems.

Education, Training and Experience (including licensure and certification)

Graduation from an accredited four-year college or university with major course work in a human service field as required to qualify as a Qualified Mental Health Professional (QMHP) Minimum of (1) year related experience is preferred. Experience may include participation as a member of an Interdisciplinary Treatment Team in developing and implementing individualized program plans for MH consumers.

Preferred experience in working directly with children and/or adolescents and their families. Knowledge base of mental illness diagnoses and behavior disorders in children and/or adolescents. Able to collaborate closely with a team of family members, community partners and professionals to develop and implement individualized plans of care. [AR1] Flexible schedule to meet with the family teams at times that are convenient to the team, to promote active participation of all wraparound team members. Required to complete State wraparound trainings: Introduction to Wraparound; Engagement in Wraparound, and Intermediate Wraparound training within the first year of hire.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

  • All NWIC Wraparound facilitation training must be completed within one (1) year of hire.
  • Possession of a valid driver’s license and current automobile insurance.

Individuals with an out of state driver’s license must be able to obtain a driver’s license in the state of Texas within thirty (30) days.

  • Successful completion of all position-specific training within thirty (30) days of employment is required.
  • The position requires adherence to all applicable laws, regulations, policies, procedures and/or guidelines of HHSC and DCMHMR. Other specific laws, regulations, policies, procedures and/or guidelines which must be followed include Texas Administrative Code, National Wraparound Implementation Center Guidelines and Case Management guidelines.

  • Ability to make decisions based on education and or clinical experience, which reflect consistency with Center’s Guiding Principles, Policies and Procedures, and Standards of Care.
  • Wraparound Care Coordinators must possess a minimum of B.A or B.S. degree in social work, psychology or related field with extensive experience in human services. Experience can be substituted for education.
  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.
  • Ability to analyze complex information, and to define and solve problems.
  • Ability to display regular and reliable in-person attendance.
  • Ability to work effectively in a team environment.
  • Understanding and practice of Trauma Informed Care principles.

Essential Duties

Below is a list of essential duties of the position. Other duties may be assigned as needed.

  • Attends and participates in all scheduled team meetings.
    • Staff will attend meetings as scheduled and sign in for meetings to acknowledge their presence and understanding of the topics discussed.
    • If a meeting is missed, staff will get with supervisor within one week to review the information.
  • Provides courteous, professional services at all times.
  • Maintains professional boundaries.
  • Interacts with center staff, individuals, and community members in a professional manner.
  • Has the ability to work independently and make decisions but seeks guidance when needed.
  • Participates in the Center’s Zero Suicide Initiative:
  • Carries a Mental Health caseload for Denton County.
    • Intensive Case management (10 max or more when permitted by HHSC)
  • Ensures each individual served are provided services as required by the level of care assignment for each month.
    • Engagement measure with consumers is at 65.6% on a monthly basis.
    • Maintains contact standards as defined in Texas Resilience and Recovery (TRR) guidelines.
    • Organizes and facilitates monthly meetings with families and their support system.
  • Administers clinical assessments and paperwork to determine appropriateness of level of care assignments required to meet individual’s needs.
    • Required paperwork includes: CANS, Wrap Plans, Recovery Plan, Substance Abuse Needs Inventory, Rights Review, Family Story, Need Statements, and consent forms.
  • Is familiar with all applicable Texas Administrative Code (TAC), fidelity tools and Texas Human Health Services (HHSC) contract information and Center policies and procedures.
    • All documentation is completed within two business days, to meet or exceed 95% monthly.
  • Ensures individuals charts are in compliance with HHSC contract requirements and standards.
    • Responds to all chart audits by assigned deadline.
    • All paperwork will be signed in black ink with credentials and date.
    • Provides emergency/crisis intervention when necessary according to HHSC and Center policies and procedures.
  • Completes other duties as assigned.
  • All assignments given are completed in a timely manner.

Assists individuals served during a crisis episode by ensuring they are linked to appropriate Center programs (MCOT, Psych Triage, Crisis Prevention Liaison, etc.).

Remains with individuals, if safe to do so, until appropriate staff can arrive to intervene.

Debriefs with immediate supervisor or designee as needed after a crisis situation.
